Our client is seeking a dedicated and experienced Physical Therapist to join a multidisciplinary home health care team. This full-time role offers the opportunity to work independently while delivering high-quality, patient-centered care in the home setting. The Physical Therapist will play a key role in helping patients regain independence, improve mobility, and enhance overall quality of life.
Key ResponsibilitiesConduct comprehensive patient assessments to evaluate functional status and clinical needs.
Develop and implement individualized care plans tailored to patient goals.
Provide therapeutic treatments and interventions within the patient’s home environment.
Collaborate with patients, families, and interdisciplinary healthcare team members to achieve optimal outcomes.
Monitor, document, and evaluate patient progress, modifying treatment plans as clinically appropriate.
Educate patients and caregivers on therapeutic exercises, mobility strategies, and functional improvement techniques.
Ensure compliance with all regulatory, professional, and organizational standards of care.
Required QualificationsActive state licensure as a Physical Therapist.
Minimum of nine (9) months of experience as a licensed Physical Therapist.
CPR certification.
Valid driver’s license with reliable transportation for travel to patient homes.
Strong communication and interpersonal skills.
Ability to work independently and manage a flexible schedule.
Excellent organizational skills and attention to detail.
Preferred QualificationsPrior experience in a home health or community-based care setting.
Compensation & Work ModelFull-time, field-based home health position.
