Hospitalist Physician

Locum Tenens NP/PA-C - Remote (California) supporting Hospital at Home patient acquisition in California. Join an innovative hospitalist telemedicine team delivering acute hospital-level care at home.

• Position Type: Locum tenens (contract) - California locum tenens job
• Location: Remote within California, with 2 days of in-person training in Walnut Creek, CA (94598)
• Estimated Weekly Salary: $6,780-$8,030/week
• Shifts per Week: 7 (telemedicine hospitalist schedule)
• Shift Duration: 10-hour days (12:00pm - 10:00pm or 7:00am - 5:00pm PST)
• Schedule: Full-time, 7 days on / 7 days off
• Start Date: May 25, 2026
• Duration: 53 weeks (with potential for extension)

• Master's degree from an accredited Nurse Practitioner (NP) or Physician Assistant (PA) program
• Active California state licensure as a Nurse Practitioner (NP) or Physician Assistant (PA-C)
• Certification: NCCPA (for PA-C) or Acute Care NP certification preferred
• Valid DEA license
• Must reside in California (state-based licensure and telemedicine regulations)
• Minimum of 5 years of clinical experience in internal medicine, emergency medicine, or as a hospitalist
• Experience with telemedicine / virtual patient care preferred (hospitalist telehealth experience a plus)
• Proficiency with electronic health records (EHR), especially EPIC
• Strong clinical assessment, communication, and coordination skills for virtual care

• Perform comprehensive virtual assessments to evaluate patients' medical conditions, history, and current symptoms
• Review and interpret medical records, labs, and imaging to guide clinical decisions
• Determine patient eligibility for the Hospital at Home program using established clinical criteria
• Develop and implement individualized care plans in collaboration with a multidisciplinary hospitalist and home health team
• Monitor and manage patients remotely, coordinating timely in-home services and follow-up with home health providers
• Provide ongoing virtual follow-up assessments and adjust care plans based on clinical status
• Communicate clearly with patients, families, and caregivers to educate and ensure understanding of at-home care plans
• Coordinate care transitions with primary care physicians and specialists to maintain continuity of care
• Document visits and maintain accurate, up-to-date patient records in the EHR in compliance with regulatory requirements

• Work in a leading Hospital at Home program using telemedicine and virtual care technologies
• Remote locum tenens opportunity within California with short in-person training in Walnut Creek
