* Main responsibilities:

* Schedule member appointments for annual or change-based functional eligibility re-determinations, optimizing time and travel resources.
* Use the DHS screening tool to assess members' functional abilities, health, and required supports.
* Collaborate with staff to ensure assessment data aligns with internal records.
* Conduct in-home, face-to-face interviews and enter assessment data into the designated online system to determine level of care.
* Submit documentation promptly and accurately according to the established guidelines.
* Attend meetings and training to stay updated on processes, screening tools, and state directives.
* Participate in quality assurance activities to maintain screening competence and reliability.
* Maintain required certification through ongoing skills testing, per State of Wisconsin standards.
* Demonstrate professionalism and respect in documentation and assessments, preserving screen integrity#

**** Required Qualifications
*** Four-year bachelor's degree in a Health or Human Services related field with one (1) year of experience working with at least one of the Family Care target populations OR be a Wisconsin licensed Registered Nurse with one (1) year of experience working with at least one of the Family Care target populations.
* Long Term Care Functional Screener (LTCFS) certification-Selected individual must meet requirements and successfully complete online screener certification training course(s) and become certified as a functional screener. The Family Care target group population is defined as: frail elders and adults with intellectual, developmental, or physical disabilities

** The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting.
