Join DaVita Integrated Kidney Care (IKC) as a Care Coordinator - Nurse Practitioner Primary Care Pilot The OpportunityIt takes a village to care for our patients, and as a Care Coordinator (CC), you'll be at the forefront of healthcare innovation. This is not your standard scheduling role; you will be a key player in a specialized program where Nurse Practitioners (NPs) serve as independent, “primary care-lite” providers for complex CKD and ESKD patients.

Location & Logistics- Role Type: 100% Remote.
- Time Zone:
Must be based in or operate within the Central Time Zone to align with the Lincoln, NE and Des Moines, IA markets. - Schedule:
Full-Time, Monday-Friday. Daily start time between 7:00-8:00 AM. - Home Office Requirements:
Quiet, private space with high-speed internet.
- You will be the primary scheduler and administrative partner for our NPs, ensuring the patient journey from intake to completion of the visit.
- Strategic Scheduling:
Own the scheduling process for NP patient panels, balancing complex schedules, facilitating outreach, and optimizing NP time. - Care Coordination:
Assist patients through the continuum of care and identified care pathways under NP guidance. - Communication Hub:
Act as primary liaison between patients, the NP, and partner nephrology office, managing inbound/outbound communications. - Innovation Partner:
Participate in workflow optimization, refine processes, and improve patient experience. - Documentation & Compliance:
Maintain accurate, real-time documentation in the EMR to ensure care teams have necessary information. - Resource Navigation:
Identify and coordinate resources, referrals, and appointments with the broader care team.
- High School Diploma or GED required.
- 2+ years of experience in a professional office or clinical environment.
- Tech-Savvy:
Proficiency in Microsoft Office (Excel, Outlook) and experience with Electronic Medical Records (EMR). - Innovator Mindset:
Excited about care redesign, comfortable with ambiguity and growth. - Excellent Communication:
Communicate with clarity, empathy, and professionalism with patients, providers, and office staff. - Autonomous & Collaborative:
Self-starter who works well remotely, and values being a visible, communicative member of a small, agile team. - Discretion:
Ability to handle confidential patient information with highest integrity.

Genuine interest in nephrology, dialysis, or primary care; excited about improving health outcomes for CKD and ESKD patients. - Clinical Office
Experience:
1-3 years in a medical office, particularly in scheduling, referrals, or care management. - Clinical Exposure: CNA, MA, or similar clinical background is a major plus.
