Aging in Place Specialists (AIPS) is a locally PT owned and operated Outpatient Private Practice (Med

B) specializing in home-based geriatric care. It's home care without the OASIS documentation! We use WebPT for our home-based outpatient EMR.

We are seeking a Full or Part Time Physical Therapist (Independent Contractor) for Sussex and Kent County Delaware to provide services to improve functional independence and wellness. Each team member is an integral part of our culture in empowering our community to live a long, healthy, independent life.

Responsibilities:

• Evaluate and create a plan of care for patient
• Administer treatment to patient
• Discharge planning of patient
• Patient Re-assessments
• Effective communication with patients physicians and healthcare team members
• Smiling (cuz it's our favorite)

• Preferably at least one year of experience as a Therapist
• Experience in geriatric care
• High proficiency and comfortable with manual therapy
• Able to provide proof of valid driver's license and reliable transportation
• Must have a portable computer (iPad, lap top) for documentation purposes
• Valid documentation with ability to work in US
