Hiring Range: $37-$45/hr
* This is a full-time/other (30 - 40 hours per week) salaried position. This position is eligible for Eagle County benefits!

This is a great position for new graduates who would like to further their training and certifications or for seasoned nurses who are tired of the crazy schedule and stress that can come along with many nursing positions. Our nurses can develop and foster long-term, therapeutic relationships with clients while having autonomy and full control over their daily schedule. Some remote work is possible.
The Nurse Home Visitor (NHV) is a member of the Healthy Families team at Eagle County Public Health and Environment (ECPHE). The NHV delivers high-quality, evidence-based services primarily through the Nurse-Family Partnership model while serving as a flexible clinical resource for ECPHE. In addition to home visiting, this role collaborates on broad community health initiatives and responds to evolving public health needs as they arise.
Bilingual (Spanish & English) fluency is required.

The schedule will be flexible and can be based on the applicant's preferred schedule- 30 to 40 hours per week, generally between Mon-Fri. Some remote work may be available; however, the position is expected to travel to clients' homes throughout Eagle County and will be required to work in the Eagle office weekly.

Responsibilities
* Provide comprehensive in-home and telehealth nurse visits to eligible women and families, adhering to the specific protocols, visit guidelines, and performance standards of applicable home visitation models.
* Manage an assigned caseload and maintain required scheduling slots to ensure timely visits for new families in accordance with program-specific protocols.
* Conduct comprehensive health and wellness screenings for prenatal/postpartum clients and infants, identifying clinical, emotional, and social needs to to develop individualized family support plans.
* Develop trusting, therapeutic relationships with families using concepts of reflection and motivational interviewing to support them in attaining targeted health and self-sufficiency goals.
* Collaborate with families to develop and implement individualized support plans, providing education and interventions that improve parenting skills, reduce stress, and promote family health.
* Act as a liaison between families and community partners, facilitating referrals and connections to resources.
* Maintain accurate, timely, and confidential client records in compliance with HIPAA regulations and program-specific nursing documentation systems.
* Actively participate in weekly team meetings, clinical case conferences, and reflective supervision to meet competency requirements and ensure high-quality program delivery.
