• Salary Range: $50,000 – $55,000

• Conduct community outreach with members and enroll them in Reema’s program.
• Provide high quality and meaningful support to members to achieve improved health and wellbeing outcomes.
• Establish trusting relationships with members, reliably provide meaningful and quality support, and follow through in a timely and appropriate manner to achieve their wellbeing goals.
• Through cold outreach, build and handle a caseload of 100–125 members and provide coverage for other Community Guides during periods of absence or increased workload.
• Understand overall goals and metrics for the month. Independently organize and prioritize workload with effective time and task management skills to achieve monthly goals and outcomes.
• Actively listen, empathize, and appreciate the vulnerability of members willing to share their challenges. Maintain professional boundaries and confidentiality, and seek consultation for complex situations.
• Utilize phone, text, and email appropriately to match each member’s communication preference.
• Effectively identify and connect members to community and health resources. Build relationships with resources to support Reema’s members as needed.
• Prepare members for graduation by keeping self‑sufficiency and independence top of mind.

• Use the Reema App to document all work with members in a timely fashion.
• Understand and comply with confidentiality of protected health information and HIPAA requirements.

• Work independently and collaboratively in a fast‑paced startup environment; view change as an opportunity.
• Serve diverse members in your community with dignity and respect, and build relationships with community resources.
• Partner with Reamites across the company to offer perspective and learn from others.

• Bachelor’s degree in Social Work, Human Services, Psychology, Health Sciences, or related field, or equivalent experience.
• Two or more years of direct experience with a target population or community and knowledge of local resources.
• Knowledge of crisis planning, case management, boundaries, and personal safety.
• Proficiencies in critical thinking, time management, strategizing, and multitasking.
• Strong motivation toward achieving goals, metrics, and targets.
• Strong propensity toward building and maintaining relationships.
• Ability to initiate conversation and create connection quickly.
• Ability to develop, adapt, and execute outreach plans.
• Basic knowledge of Microsoft Word, Outlook, Excel, video conferencing; G‑Suite preferred.
• Technology proficient.
