Speech Pathologist
Speech Therapist, is responsible for providing rehabilitation services for home based clients in need of speech therapy to assist patients in communicating and swallowing appropriately, including assessing patients, diagnosing disorders that can impact swallowing and speaking and providing treatment for their patients
Primary Functions:
- Visits patients in home/hospital. Performs initial and ongoing assessments, including assessments at appropriate time points and evaluates the level of function and identifies problems.
- Develops initial and ongoing treatment plans, including goal setting
- Provides treatment in accordance with physician orders.
- Utilizes equipment required to increase the client's function and independence.
- Re-evaluates patient as appropriate.
- Revises plan as necessary.
- Prepares clinical and progress notes in a timely manner.
- Provides for discharge planning and communicate plans to case manager.
- Consults with clients and families and VNA staff as appropriate.
- Participates in Quality Improvement activities.
- Makes appropriate interorganization and intraorganization referrals.
- Participates in in-service programs and attends meetings as required.
- Familiarizes and complies with all VNA policies and procedures.
- Meets standards of behavior expectations.
- Follows established guidelines for use and/or disclosure of protected health information. Employees should report any breaches of the Health Insurance Portability and Accountability Act (HIPAA) rules to the Privacy Officer.
- Other duties as assigned.
Compensation: $50.00 to $53.00 per hour,depending on experience.
Available Employee Benefits: 403b (Only eligible if 1,000 or more hours worked per year)
Qualifications
Bachelor’s degree with a major in communications disorders and sciences.
2. Licensed in the State of Illinois as an Speech Therapist.
3. BLS Certification
4. Health policies and requirements met (see personnel policies).
5. Three years experience as Occupational Therapist preferred.
Essential Functions/
Physical Requirements:
1. Ability to perform tasks involving physical activity which includes heavy lifting and extensive bending and standing.
2. Ability to communicate effectively, verbally and in writing.
3. Tactile, visual, auditory, and olfactory senses intact.
4. Ability to climb stairs and ambulate in and out of patient’s homes in all weather.
5. Ability to drive a car.
6. Ability to deal effectively with high levels of stress.
7. Ability to wear a particulate respirator during patient care as necessary.
