Description and Responsibilities
The Intensive Home-Based Therapeutic Care program provide services for youth with emotional challenges and/or multistressed families. The scope of services includes flexible individualized treatment, rehabilitation, supervision, and support that vary in intensity, depending on youth and family needs. The services are delivered by a core team of staff using a Wraparound Philosophy support which helps family, support members, and service providers work together to support the family or child.
In addition, all services are delivered using a Trauma Informed Care approach.
The Young Adult Peer Mentors use their lived experience to support young adults with setting and implementing goals in education, employment, housing, relationships, life skills and social needs. Mentors also help peers in identifying strengths and interests to assist in monitoring interventions and personal recovery goals.
OtherKey Responsibilities
- Facilitate the development of valued community roles including group memberships, employment, or volunteer experiences.
- Support the development and maintenance of key skills including stress management, relapse prevention, self-determination, self-advocacy, decision making, and goal planning using recovery tools.
- Assist peers in the development of a health and wellness plans including relapse prevention, planning, and understanding the symptoms of mental illness.
- High School Diploma, GED or equivalent, required.
- Experience living with and recovering from mental health challenges, substance misuse, and or traumatic life challenges, required.
- At least 1 year of experience working with youth and or young adults, required.
- Valid Drivers License, acceptable driving record and reliable vehicle, required.
