Job Summary and Responsibilities

Acentra Health is looking for a Clinical Assessor to join our growing team. This role is hybrid, based within Anson or Union County of North Carolina, and involves working from a home office and traveling up to 60 miles. The assessor will complete needs‑based eligibility determinations for North Carolina Medicaid beneficiaries under the Personal Care Services (PCS) and Community Alternatives (CAP) programs, conducting assessments in patients’ primary residences and making level‑of‑care recommendations.

  • The PCS Assessor determines eligibility for Medicaid‑funded personal care services for beneficiaries with medical conditions, cognitive impairment, or disability.
  • The CAP/PCS Assessor completes assessments for community‑based waivers to keep beneficiaries in their homes or community settings.
  • Assessments are generally performed in the beneficiary’s primary residence.
  • Travel up to a 60-mile radius is required.
Required Qualifications
  • Registered Nurse or LCSW license in the applicable state.
  • Minimum two years of nursing experience.
Preferred Qualifications
  • Experience with community‑based individuals needing personal assistance with ADL and IADL tasks.
  • Experience conducting PCS assessments.
  • 2+ years of home healthcare experience.
  • 2+ years of directly related case‑management experience in the health or medical field.
  • Experience conducting HCBS waiver assessments.
  • Knowledge of North Carolina Medicaid Clinical Coverage Policy (Clinical Policy) 3L and PCS Program Provider Manual.
  • Knowledge of standards of practice related to Medicaid‑funded personal care services, HCBS programs, and EPSDT.
  • Knowledge of public sector services and supports.
  • Computer proficiency in Microsoft Excel, Word, and Outlook.
  • Ability to use computer equipment and web‑based software to conduct work.
  • Ability to collaborate with office staff, healthcare professionals, patients, families, and other supports.
  • Strong communication skills, written and electronic media.
  • Attention to detail, ability to prioritize tasks effectively and execute them timely.
  • Ability to work independently with minimal supervision.
  • Develop level‑of‑care recommendations based on clinical evaluations.
  • Participates in training of PCS and CAP stakeholders as needed.
  • Person‑centered thinking and competency in awareness of needs of persons with disabilities.
  • Knowledge of North Carolina Medicaid Clinical Policy 3K-1, 3K-2, and 42 CFR Part 441 Subpart G.
  • Knowledge of eligibility criteria for LOC and waiver participation.
  • Knowledge of standards of practice related to Medicaid waivers, HCBS programs, EPSDT, medical fragility, and level‑of‑care determinations.
Compensation

USD $77,000.00 – $80,000.00 per year.
