Job Title
Ambulatory Care Clinical Pharmacist
Location & FlexibilityRemote work flexibility offered; preference for candidates located in or near Tampa, Lakeland, Sarasota, or Polk County, Florida, who are available for onsite visits as needed to support care team collaboration and community engagement.
Responsibilities- Provide Comprehensive Medication Management (CMM) in collaboration with hub-based and virtual clinical care teams.
- Develop and monitor medication‑management action plans to achieve member self‑management goals and clinical outcomes, with measurable parameters and timelines for follow‑up.
- Communicate and work collaboratively with Cityblock and external providers to ensure all clinical interventions and encounters are aligned with medication‑related goals; conduct ongoing assessments and refinements to care plans.
- Offer direct support to clinical team members for general drug information, dosing, interaction/adverse effect avoidance, and member‑specific needs.
- Participate in staff education through in‑services, development of educational tools for clinical and non‑clinical team members, and training/coaching/facilitating all roles on maintaining a member’s best possible medication list via the Commons platform and post‑discharge medication reconciliation process.
- Help develop medication‑management and disease state‑specific clinical pathways/protocols, with the eventual goal of transitioning to collaborative practice agreements (CPA) that allow pharmacists to perform assessments, order laboratory tests, and initiate or adjust medications per protocol.
- PGY1 pharmacy practice residency plus a PGY‑2 Ambulatory Care Pharmacy Residency, ora minimum of 2 years’ experience as a clinical pharmacist in an ambulatory care setting.
- Florida pharmacy license required.
- Spanish language proficiency strongly preferred.
Must complete Cityblock’s medical clearance requirements, including evidence of immunity to MMR, HepatitisB, Varicella, and a TB screen, or have an approved medical or religious accommodation that precludes vaccination against these diseases.

CompensationExpected salary range for this position: $135,000–$150,000.
