Pharmacist, Clinical​/MTM Remote, PST at Molina Healthcare Chandler, AZ

Pharmacist, Clinical/MTM – Remote (PST) – Molina Healthcare. The position is fully remote and runs Monday through Friday, 9am to 5pm Pacific Standard Time. The pharmacist will perform telephonic, targeted and comprehensive medication reviews, focusing on PartD STARS, adherence, Medicare/MMP LOB and other cost‑effective medication use initiatives.

• Provide medication therapy management, including comprehensive medication reviews and communication with members and providers.
• Examine member medical records, apply evidence‑based medicine and national guidelines, and create recommendations to optimize medication regimens.
• Analyze medication lists for potential patient safety problems such as drug interactions or suboptimal regimens.
• Promote clinically appropriate prescribing practices through provider profiling, member drug profile reviews, medication protocols, and case‑by‑case interventions.
• Perform prior authorization reviews of high‑cost medications and deferred authorization requests.
• Research, develop, and implement drug utilization and disease‑state management strategies to deliver high‑quality, cost‑effective care.
• Coordinate pharmacy‑related care with members, providers, and interdisciplinary teams, including referrals to state plan health care services.
• Follow up with members and providers to monitor progress toward drug treatment goals.
• Serve as a drug information resource for pharmacy staff, medical affairs staff, and providers.
• Review and analyze pharmacy benefit manager reports to identify trends and opportunities for targeted utilization management.
• Collaborate with Health Care Services, data managers, and case managers on member‑centered interdisciplinary teams.
• Prepare drug monographs and utilization reports for the Pharmacy & Therapeutics Committee.
• Assist the Director in developing/updating policies and procedures and implementing changes to comply with state and federal regulations.

• Doctor of Pharmacy; a Bachelor’s Degree in Pharmacy plus ten years of relevant experience, including clinical work, will be considered in lieu of a Pharm.

• ASHP‑accredited residency or two to three years of relevant pharmacy experience.
• Current knowledge and expertise in clinical pharmacology and disease management.

• Active and unrestricted State Pharmacy License for the workplace and plan location.

• Ambulatory Care Residency

• Experience with Medicaid and/or Medicare programs.
• Clinical skills or pharmacotherapy expertise related to MTMP‑related conditions and medications.
• Experience with team‑based care.

• Board Certified Ambulatory Care Pharmacist.
• American Pharmacist's Association Medication Therapy Management Certificate Training Program certificate or equivalent.

Pay Range: $80,412 – $156,803 / ANNUAL (Actual compensation may vary based on geographic location, work experience, education, and/or skill level).
