Schedule:

3 weekdays, 8:30 am to 5:00 pm. Territory:
Fayette, Ross, Pickaway, Hocking, Vinton, Jackson, Pike and Highland Counties. Agency:

You’ll provide home-based skilled speech therapy care for patients as directed by the attending physician, educate patients and their families, and ensure patient safety.

• Plan of Care:
  Develop an individualized ST plan of care and make adjustments as needs change.
• Guide the patient in use of therapeutic, creative and self-care activities to improve functioning.
• Observe, record, and report patient reactions, changes, and progress to the attending physician and appropriate agency staff.
• Use activities and home exercise programs to improve swallowing, breathing, cognition, speech, and communication, and/or other speech therapy related functions.
• Help decrease re-hospitalizations by teaching preventive measures and promoting good decision-making.
• Act as a Family Educator/Advocate, counseling family on the disease/injury and how to manage in all phases of speech therapy.
• Complete all clinical documentation following agency protocol and Medicare/Federal guidelines.
• Understand and follow agency policies, procedures, rules, and regulations, and communicate changes in schedule/availability to schedulers or supervisors.
• Attend in-service training and mandatory agency meetings.

• Compassionate communicator with a positive attitude.
• Patience when working with patients, families, physicians, and coworkers.
• Attention to detail and observant follow-through.

• Speech Therapist with a current license in the state of employment.
• Minimum one-year experience as a ST in an acute care setting.
• Home care experience preferred.
• Valid driver's license and auto insurance in your name as a driver.
• Capable of all physical demands of the role.
