• Provide patient care in a variety of settings including traditional residential homes, assisted and skilled facilities, hospitals, and the hospice care center throughout Mesa County, Grand Junction, CO.
• Make symptom management & urgent visit needs and attend after-hour deaths for all Hope West service lines.
• Quickly assess the patient/family situation and possible interventions.
• Communicate thoroughly and specifically to the audience.
• Work from home between patient visits.
