Minimum Requirements- Doctoral degree in Physical Therapy (DPT) from an accredited program approved by the APTA.
- Active Washington State physical therapy license required to start the assignment.
- BLS (CPR) certification required.
- Home health experience required or strongly preferred.
- Experience with SOC, evaluations, ROC, recertifications, and subsequent visits.
- Experience with HCHB EMR preferred.
- Willingness to cover patients within approximately a 50 mile radius.
- Business casual dress code adherence.
- Previous home health agency experience preferred.
- Familiarity with LHC or similar home health documentation and coding guidance preferred.
Shift hours: 08:00 - 16:30 (days). Weekly guaranteed hours: 40. On-call or weekend requirement:

ResponsibilitiesProvide home-based rehabilitative care including start-of-care evaluations, ongoing skilled therapy, recertifications, and discharge planning. Use clinical judgment to develop and implement individualized treatment plans that support safe functional mobility and independence. Communicate with interdisciplinary team members and document care according to agency and regulatory standards.
