Position Overview

Signature Healthcare at Home is seeking a compassionate and skilled Physical Therapist (PT) to join our Home Health team on a PRN/hourly basis in Lincoln City, OR. This role offers flexibility and the opportunity to make a meaningful impact by providing patient-centered care in patients’ homes along the scenic Oregon Coast. The ideal candidate is independent, patient-focused, and passionate about delivering high-quality rehabilitative care to diverse populations.

• Conduct comprehensive physical therapy evaluations in the home setting.
• Develop and implement individualized plans of care based on patient needs and physician orders.
• Provide hands‑on therapy to improve mobility, strength, balance, and functional independence.
• Educate patients and caregivers on treatment plans, exercises, and safety strategies.
• Accurately document visits, progress, and outcomes in the EMR system.
• Collaborate with interdisciplinary team members including nurses, OT, SLP, and physicians.
• Monitor patient progress and adjust treatment plans as needed.
• Ensure compliance with all regulatory, ethical, and company standards.

• Graduate of an accredited Physical Therapy program.
• Current Oregon Physical Therapist license (or ability to obtain).
• Minimum of 1 year of experience preferred (home‑health experience a plus).
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal skills.
• Reliable transportation, valid driver’s license, and auto insurance.
• Ability to work independently and manage a flexible PRN schedule.

PRN / Per Diem / Hourly – flexible scheduling based on availability and patient needs. Competitive hourly rate (DOE). Mileage reimbursement available.
