Schedules: Monday – Friday (PST & MST Time Zone)
Location: Remote – Work From Home
This role is 100% remote with no face‑to‑face contact with patients. We have a “remote first” environment and are proud to have team members located nationally.
As a Continuous Care Coach, you will assess, coordinate, and manage the care needs of patients with chronic eye diseases. You will work closely with patients, their families, healthcare providers, and internal teams to develop and implement personalized care plans. Your primary goal is to ensure patients receive high‑quality care and support throughout their journey, interacting with up to 15 patients per day to provide encouragement, empathy, and support.
EssentialFunctions
- Manage the patient caseload and communicate as frequently as necessary based on patient needs.
- Engage patients, families, caretakers, and other professionals in care communication.
- Coordinate with home health agencies and community services.
- Collect health outcomes data and maintain registry documentation.
- Educate patients and families for self‑management and daily living.
- Support treatment regimen adherence and medication management.
- Identify available community and health resources.
- Facilitate access to required care and services.
- Review patient status, including laboratory and other studies.
- Develop, communicate, and maintain comprehensive or disease‑specific care plans.
- Use motivational interviewing to assess and address barriers to care.
- Work flexibly within normal business hours of the patient’s time zone.
- Communicate regularly with Team Leads, Managers, Directors, and stakeholders.
- Perform other duties as assigned to support Lumata Health’s goals.
- Work independently in a team environment with minimal supervision.

- Ability to provide guidance and insights to patients.
- Work across multiple time zones and maintain reliable connectivity.
- Consistently participate in phone conferences as required.
Education and Experience
- 2+ years of work experience in eye care preferred.
- Must have an active Certified Ophthalmic Assistant (COA) or Certified Ophthalmic Technician (COT/COMT) designation.
- Proficiency with Electronic Medical Record (EMR) systems preferred.
- Maintain a HIPAA‑compliant home office.
- Prolonged periods of sitting while working on a computer.
- Ability to hear, speak, and virtually meet.
- Strong verbal and written communication skills.
To perform essential functions, you must have a dedicated remote workspace with a minimum of 50 Mbps download and 10 Mbps upload speed, a private and quiet environment, and a company‑issued laptop/PC that ensures only you can access patient data. Your camera should be on during virtual meetings.
