We’re looking for a Care Manager to join our Long-Term Services and Supports (LTSS) program, supporting members through a mix of field-based visits and remote work
.
Schedule
:
Monday-Friday, 8am-5pm CST (Flexible scheduling for field visits)
Requirements
- 3–5 years of clinical experience
- 2–3 years in case management, discharge planning, or home health
- Strong communication, documentation, and organizational skills
- Ability to work independently and travel locally
Day-to-Day
- Complete comprehensive member assessments and individualized care plans
- Coordinate care across providers, services, and community resources
- Manage waiver services (in-home care, emergency response systems, etc.)
- Collaborate with members, families, and care teams to address complex needs
- Serve as a liaison to ensure seamless, high-quality care delivery
