Job OverviewWe are currently seeking a compassionate and skilled Home Health Speech-Language Pathologist to provide personalized, one-on-one therapy in a home setting throughout Christiansburg and the surrounding areas. This position is ideal for a Speech-Language Pathologist who enjoys working independently while helping patients improve communication, swallowing, and cognitive function within their daily environment.
As a Home Health Speech-Language Pathologist, you will work directly with patients recovering from illness or injury while developing individualized treatment plans focused on improving patient outcomes and quality of life.
Responsibilities- Provide home-based speech therapy to patients experiencing communication, swallowing, or cognitive difficulties
- Conduct patient assessments and develop individualized treatment plans
- Observe and evaluate a patient’s ability to communicate, swallow, and process information
- Monitor patient progress and modify care plans as needed
- Assist patients with mouth, breathing, swallowing, fluency, articulation, and reading exercises
- Educate patients and families on therapy goals, safety practices, and self-care techniques
- Collaborate with interdisciplinary team members including RN, LPN, CNA, HHA, PT, and OT
- Maintain accurate and timely documentation of patient progress
- Master’s or Doctoral degree in Speech-Language Pathology
- Active SLP license in the state of Virginia
- Minimum of 2 years of speech therapy experience (home health preferred)
- CPR certification required
- Knowledge of state and federal home health regulations
- Strong clinical judgment, critical thinking, and communication skills
