Registered Dietitian Nutritionist - PT, Hybrid, Remote in Merced

Job Overview

We’re looking for a Registered Dietitian (RD/RDN) who wants more than just a routine role, someone who’s ready to bring clinical expertise, collaboration, and real impact to post‑acute care settings.

• Perform comprehensive nutrition assessments and ongoing follow‑ups
• Develop and implement individualized nutrition care plans
• Collaborate with nursing, leadership, and care teams
• Support compliance with CMS and state regulations
• Participate in care conferences and survey readiness efforts
• Provide guidance, education, and support to facility staff

• Active Registered Dietitian (RD/RDN) credential
• Must have experience in skilled nursing, long‑term care
• Strong understanding of regulatory standards (CMS/Title 22 a plus)
• Self‑motivated, organized, and confident working independently
• Reliable transportation for onsite visits
