Job Overview
We are looking for a RN Health and Social Services Care Coordinator to work in the field coordinating long‑term care needs for patients in Oahu, HI. The role is part of an Integrated Care Model and involves advocating for patients and ensuring they receive appropriate services.
Primary Responsibilities- Assess, plan and implement individualized care strategies that direct patients toward the most appropriate, least restrictive level of care.
- Identify and initiate referrals for social service programs including financial, psychosocial, community, and state supportive services.
- Manage the care plan throughout the continuum of care as a single point of contact.
- Communicate health‑related information to all stakeholders to ensure coordinated care and timely delivery of services.
- Advocate for patients and families so their needs and choices are fully represented and supported by the health care team.
Location:
Oahu, HI. Remote work is an option for candidates located on Oahu; however, travel to local sites may be required up to 75% of the time. All employees working remotely must adhere to United Health Group's Telecommuter Policy.
- Current, unrestricted RN license in the state of Hawaii.
- 2+ years of clinical experience.
- Experience working directly or collaborating with services for long‑term care, home health, hospice, public health or assisted living.
- Intermediate proficiency with MS Word, Excel and Outlook.
- Ability to travel within the assigned region to visit Medicaid members in their homes or other settings.
- Driver's license and reliable transportation.
- Current access to, or ability to obtain, reliable internet access via a landline.
- Bachelor's degree or higher.
- CCM (Certified Case Manager) certification.
- Experience with discharge planning, utilization review, concurrent review, or risk management.
- Experience with electronic charting and arranging community resources.
- Field‑based work or case management experience.
- Experience in community health settings involving behavioral/mental health, psychiatric nursing, housing/homeless outreach, pregnancy, pediatrics or disease management.
- Experience managing populations with complex medical or behavioral needs or serious mental illness.
- Bilingual in Cantonese or Mandarin.
- Background in community nursing such as PHN or IP psych facility.
- Hourly pay ranges from $28.27 to $50.48 based on full‑time employment.
