Physical Therapist – Home Health

Location: Montgomery, Giles, Floyd, Pulaski counties and surrounding areas

Position Type: Regular PRN (< 20 hours/wk)

Agency: Warm Hearth at Home

Pay: Hourly $55 – $65 depending on experience; $0.65 per mile mileage reimbursement

Warm Hearth at Home is expanding our team of Home Health physical therapists. We currently need a regular PRN Physical Therapist, with the possibility to transition to part‑time/full‑time as census grows. Our agency is staffed fully in‑house with a small, tight‑knit administrative and HR team.

• Evaluate and assess patients in the home setting and collaborate closely with PTAs to ensure individualized and effective intervention
• Establish measurable and meaningful goals with regular reassessment to ensure progress and maximize the effectiveness of therapy
• Maintain clear and effective communication regarding patient care with support staff, physicians, and family members to ensure efficient care
• Complete documentation timely, accurately, and completely

• Graduate of a Physical Therapist Education program approved by CAPTE
• Current Physical Therapist licensure in the state and American Heart Association CPR certification required; CPR certification may be offered and completed upon hire
• Driver's license and proof of auto insurance
• Experience in a home health setting preferred
• Ability and confidence to recognize red flags and/or medical emergencies in the home, and to triage effectively
