About the Role

The Health Home Coordinator for Children plays a pivotal role in ensuring that children with complex needs receive comprehensive and coordinated care. This position involves working closely with families, healthcare providers, and community resources to develop individualized care plans that address the unique needs of each child. The coordinator will facilitate communication among all stakeholders, ensuring that services are delivered effectively and efficiently.

By monitoring progress and adjusting care plans as necessary, the coordinator will help improve health outcomes and enhance the overall well‑being of the children served. Ultimately, this role is essential in bridging gaps in care and promoting a holistic approach to health management for children in the community.

Position Includes
  • Become an integral team member providing BHH services to children and families throughout the area; other team members include a nurse, psychiatrist, family support worker, and clinical team leader.
  • Statewide trainings to assist with integration of physical health and mental health care.
  • Weekly clinical group supervision with the BHH child and adolescent psychiatrist.
  • Bi‑weekly individual clinical and administrative supervision.
  • Electronic health record with remote access – work from home, office, and community.
  • Active collaboration with community providers in mental health, physical health, and other settings, including other social service agency program staff, primary care, medical specialists, schools, and working with JCCO’s or DHHS as necessary.
  • Flexible schedule – you make your own.
Salary & Benefits

Minimum annual starting salary of $45,760.00. Staff meeting performance expectations can earn over $5,500 annually with monthly incentives; starting salary contingent with experience.

  • $5,000 Recruitment & Retention Bonus: $1,500 at end of orientation, $1,500 at successful completion of probationary period, and $2,000 at 1 year employment.
  • Four (4) day work week option after successful probation completion.

Qualifications
  • Bachelor's Degree from an accredited college, with specialization in psychology, mental health and human services, behavioral health, behavioral sciences, social work, human development, special education, counseling, rehabilitation, sociology, nursing, or a closely related field.
  • Relevant experience working with families and children specifically in a behavioral health case management capacity.
  • Working knowledge of pertinent local community resources.
  • Computer skills necessary to function successfully in an electronic health record environment.
  • Must have own, reliable transportation and a valid driver’s license. Travel required covering Brewer and Fairfield regions.
