To provide essential care coordination services, the full-time LPN Care Coordinator - Remote will manage patient care plans, communicate with healthcare teams, and monitor patient progress while working from home.
Key responsibilities:- Develop and maintain individualized care plans for patients, ensuring timely management of care activities
- Conduct monthly care calls and educate patients on chronic conditions and treatment plans
- Monitor patient progress and document all relevant information in electronic medical records
- Bilingual in English and Spanish
- Minimum of two years of experience providing patient-centered care
- At least one year of experience in skilled nursing
- Proficient in working with electronic medical records
- Ability to perform nursing assessments and develop care plans
