Pay: Starting at $95,000+ (based on experience)

Schedule: Hybrid - 3 clinic days and 2 community-based days

Position Summary

Mind&Mobility is seeking an exceptional Physical Therapist with Geriatric expertise to join our growing interdisciplinary team serving older adults throughout South Florida. This is a unique hybrid position combining outpatient clinical care, home-based therapy oversight, Parkinson's disease program development, and interdisciplinary leadership.

If you are passionate about helping seniors thrive at home, preventing unnecessary facility placement, and building innovative programs for aging adults, this role offers an opportunity rarely found in traditional outpatient or home health settings. Our mission is to help seniors thrive as they age using a proactive approach utilizing PT, OT, SLP, LCSW and in home caregivers to address all of their psychosocial emotional needs.

Key Responsibilities
  • Evaluate, develop, and implement individualized physical therapy treatment plans for primarily geriatric patients.
  • Provide outpatient Medicare Part B physical therapy services with a focus on improving mobility, strength, balance, and functional independence.
  • Assess and treat patients with neurologic conditions, including Parkinson's disease.
  • Develop and implement fall prevention and mobility enhancement programs.
  • Conduct home visits for higher-acuity patients and perform supervisory visits as needed.
  • Collaborate with Physical Therapy Assistants (PTAs) and provide clinical oversight.
  • Work closely with Occupational Therapists, Speech Therapists, Social Workers, Care Managers, and caregivers to coordinate patient care.
  • Participate in interdisciplinary care conferences and communicate treatment recommendations to patients and families.
  • Educate patients and caregivers on home exercise programs, safety strategies, and functional mobility.
  • Assist in the development and growth of specialized Parkinson's and aging-in-place programs.
  • Complete timely, accurate, and compliant clinical documentation.
  • Maintain compliance with Medicare, state regulations, and company policies.
  • Contribute to quality improvement initiatives and promote evidence-based clinical practice.
Qualifications

Required:

  • Doctor of Physical Therapy (DPT)
  • Active Florida PT License
  • Minimum 3 years of clinical experience
  • Strong geriatric rehabilitation background
  • Excellent communication and leadership skills

Preferred:

  • Geriatric Clinical Specialist (GCS) Certification
  • Parkinson's disease expertise
  • LSVT BIG certification
  • Experience in Medicare Part B
  • Experience supervising PTAs
  • Program development experience
