Mentor Behavioral Health, part of the Sevita family, provides Early Intervention, Applied Behavioral Analysis (ABA) and Behavioral Health services to children, individuals and families. Services are provided in homes, communities, office and virtually – whichever best meets the needs of individuals served.

The Therapist at Mentor Behavioral Health provides therapeutic interventions to youth, families and adults.

• Conduct diagnostic evaluations.
• Provide therapy, family support, and crisis interventions to an individual, family and/or group using various treatment modalities.
• Provide mental health consultation to stakeholders and treatment team members.
• Develop individualized treatment plans in coordination with an interdisciplinary team.
• Develop and coordinate discharge planning.
• Maintain timely and accurate health record documentation.
• Attend regular staff meetings, clinical meetings and one-on-one sessions with direct supervisor to process current status of cases.
• Provide 25 service delivery hours per week.

• Bachelor's, LSW + at least 3 years’ experience
• Master’s Degree in human services + LPC, MFT
