We’re seeking a compassionate, experienced Home Health Occupational Therapist to join our high-performing team. The ideal candidate has strong clinical judgment, outstanding communication skills, and a deep commitment to patient-centered care. You’ll be part of a collaborative, supportive team that treats each other like family while delivering the highest standard of home-based therapy.

• Conduct Occupational Therapy Evaluations to establish individualized plans of care
• Evaluate patients’ functional abilities related to activities of daily living (ADLs) and instrumental activities of daily living (IADLs)
• Develop and implement personalized treatment plans to improve independence, safety, and quality of life
• Provide therapeutic interventions addressing self-care, home management, cognition, and upper extremity function
• Educate patients and caregivers on adaptive techniques, energy conservation, home safety, and use of adaptive equipment
• Collaborate closely with physicians, nurses, physical therapists, speech therapists, and other care-team members to coordinate care
• Monitor patient progress, update care plans, and maintain thorough, compliant documentation
• Ensure compliance with all Medicare, CMS, and home health standards

• Current Florida Occupational Therapist (OT) license (required)
• Previous home health experience is strongly preferred but not required
• Excellent communication, time management, and documentation skills
