HSS Clinical Coordinator – Field Based
Locations:
Washington, Marshall, Clay, Riley, Geary, & Pottawatomie, KS counties (2366883). This field position has a home-based office and requires travel up to 75% of the time to visit members’ homes, nursing homes, assisted living facilities, hospitals, community centers, and providers' offices.
Monday – Friday 8:00 am – 5:00 pm CST
Primary Responsibilities- Assess, plan, and implement individualized care strategies aimed at the most appropriate and least restrictive level of care.
- Identify and initiate referrals for social service programs, including financial, psychosocial, community, and state supportive services.
- Manage the person‑centered service/support plan throughout the continuum of care.
- Communicate health‑related information to stakeholders to ensure quality, coordinated care and services are delivered expeditiously to all persons.
- Advocate for persons and families to ensure their needs and choices are fully represented and supported by the health care team.
- Conduct home visits in coordination with the person and care team, which may include a community service coordinator.
- Conduct in‑person visits to nursing homes, assisted living facilities, hospitals, or other settings.
- Serve as a resource for the community care coordinator, if applicable.
- Bachelor's degree in Social Work, Rehabilitation, Nursing, Psychology, Special Education, Gerontology, or a related human services area.
- At least 2 years of experience working within the community health setting in a health care role.
- Minimum 1 year of experience working with persons with long‑term care needs.
- Minimum 1 year of experience working with persons receiving services under one of the home and community‑based waivers in KS.
- Minimum 1 year of experience using MS Word, Excel, and Outlook.
- Ability to travel within assigned regions to visit Medicaid members.
- Access to reliable transportation and a valid U.S. driver's license.
- Licensed Social Worker or clinical degree.
- Background in managing populations with complex medical or behavioral needs.
- Experience with electronic charting.
- Experience arranging community resources.
Pay range: $28.94 to $51.83 per hour based on full‑time employment. Other benefits include a comprehensive benefits package, incentive and recognition programs, equity stock purchase plan, and 401(k) contribution. All benefits are subject to eligibility requirements.
