The Specialty Case Manager (Bilingual Spanish/English) will be a member of our Social Services department, working to support patients at our Flagship location El Nuevo San Juan Health Center in the Bronx, NY. The Specialty Case Manager position is essential to ensuring continuity, quality, and accessibility of care for patients with complex healthcare needs. This role supports patients by coordinating home care services, providing documentation and correspondence assistance, facilitating access to medical transportation and community resources, and serving as a patient advocate.
The position strengthens care coordination, reduces barriers to services, and helps to improve patient outcomes. This role emphasizes support services within our PCU clinic, which provides comprehensive HIV health services.
- Provides case management assistance to patients, including special populations. Case management assistance includes general and specialized screenings, assessments, advocacy, and referrals to internal and external services as needed.
- Obtains specific resources that are required to improve patient health, including but not limited to Medical Durable Equipment, M11Q/Home Care Applications, Medical Transportation Requests, and other resources requested by provider.
- Generates letters for patients related to diagnosis, request for changes, and support services.
- Assists patients by completing prior authorizations for special medications.
- Provides resources for patients such as food pantries, nutrition information, housing resources, legal resources, DV programs, and health insurance assistance.
- Conducts intake and creates and updates patient charts. As part of intake, completes patient assessments to include screening for psycho-social and nutritional problems, and learning needs and provides appropriate referrals to case manager, LCSW, nutritionist, etc. as needed. Assures that patients in crisis are immediately assessed by a provider or LCSW.
- Serves as an advocate and networking professional to connect patients with services that are catered to their needs.
- Other related duties as assigned.
- Bachelor's Degree in Social Work, Psychology, Human Services, or related field of study;
Entry level. - Bilingual Spanish/English, required.
- Team-player and willing to go the extra mile to support patients in our care.
- Proficiency with Microsoft Office.
- Ability to thrive in either an onsite or remote environment.
- Commitment to excellence and providing quality service to patients.
HYBRID, Tuesdays through Saturdays 8:00 a.m. to 5:00 p.m. with Sunday and Monday OFF. Saturday shift is fully remote/work from home. Tuesdays through Fridays are stationed onsite.
Compensation$45,560 per annum based on 40 hours per week
