Overview

Support Long-Term Mobility and Independence at Home. @Home Therapy is seeking a Physical Therapist to provide in-home rehabilitation services to adult patients in the Jericho area. This role focuses on improving mobility, restoring functional movement, and helping patients maintain independence within their home environments. You’ll be working in a community-based setting where therapy is directly applied to real-life movement challenges inside the home.

• Evaluate balance, gait, strength, range of motion, and functional mobility in the home environment.
• Identify environmental and physical barriers that may affect safe mobility and independence.

• Develop and implement individualized treatment plans to improve or maintain mobility, strength, balance, and functional independence.
• Provide rehabilitation for patients recovering from illness, injury, surgery, or managing chronic conditions.

• Train patients in safe transfers, ambulation, stair negotiation (when applicable), and household mobility.
• Educate patients and caregivers on fall prevention, safe movement strategies, and the use of assistive devices.

• Monitor patient progress, reassess functional status, and modify treatment plans based on clinical findings.
• Collaborate with physicians and the interdisciplinary care team to support patient goals.

• Complete timely documentation using the agency's electronic medical record (EMR) system.
• Maintain compliance with clinical, regulatory, and agency documentation standards.

  Deliver care directly in patients’ homes with minimal clinic constraints.

• Per-Visit Compensation Model:

• Active New York State Physical Therapist license.
• Experience with adult rehabilitation preferred.
• Strong understanding of gait, balance, and functional movement training.
• Ability to work independently in a home care setting.
• Strong communication and clinical reasoning skills.

• Prefer functional, real-world rehabilitation over clinic-based treatment.
• Enjoy working independently in a mobile setting.
• Focus on long-term mobility and fall prevention.
