About the RoleWe are seeking a Registered Dietician responsible for delivering high‑quality, culturally responsive nutrition services and diabetes education to CS‑HHC patients. This role includes conducting individual counseling sessions, leading self‑management classes, and providing group presentations. The Dietician will also support the promotion of nutrition services across CS‑HHC, maintain accurate documentation of all nutrition encounters, ensure that educational materials remain current and evidence‑based, and contribute to the development and implementation of cost‑effective nutrition interventions that better meet patient needs.
There may be an opportunity for remote work.
- Provide individual nutrition counseling tailored to the specific needs of each patient, with particular attention and sensitivity to the cultural food practices of an ethnically diverse population.
- Teach classes to assist and educate patients in self‑management and monitoring of diabetes, weight management, and other disease focused dietary needs.
- Assist in development of diabetes prevention programing, general curriculum, and educational tools.
- Maintain accurate and up‑to‑date records of nutrition services through approved EMR, including timely completion of documentation and billing requirements.
- Work well independently and with interdisciplinary team, including medical and support staff, to provide services and prioritize patient care.
- Maintain status as a Registered Dietitian by regularly participating in approved CEU activities.
- Participate in peer review process.
- Performs all other duties as required.
- Education:
- Bachelor’s Degree in food and nutrition, dietetics, nutritional sciences, or human nutrition is required.
- RD/RDN credentialing and active status through Commission on Dietetic Registration. State Certification.
- If Certified Diabetes Educator: CDE certification. Credentialing through the Certification Board for Diabetes Care and Education.
- Experience:
- Completion of an accredited dietetic internship.
- Experience and training in individual outpatient nutrition care and group education.
- Certified Diabetes Educator (CDE/DCES) preferred. Experience counseling patients in diabetes self‑monitoring of blood glucose (SMBG), and teaching DSME (Diabetes Self‑Management Education).
