Join our Adult Intensive Services Division as a Certifications Case Manager and play a vital role in supporting adults with serious and persistent mental illness on their path to stability and independence. In this dynamic position, you’ll partner closely with our post-hospital and Hospital Liaison teams to coordinate care for clients certified for outpatient treatment, ensuring they remain engaged in services and compliant with court-ordered processes.

You’ll serve as a critical bridge between clients, clinical teams, and the legal system—delivering and filing certification paperwork, supporting court petitions, managing documentation, and maintaining accurate tracking systems. This role offers a meaningful mix of office, remote, and community-based work, ideal for someone who is highly organized, detail-oriented, and passionate about client recovery. Schedule

This position will work Monday - Friday, 8:30 am - 5:00 pm. It is eligible for a hybrid model that allows for a mix of remote and in‑office work, with an expectation of being in the office two days per week and working from home three days. Mondays and Fridays are in‑office. Tuesday through Thursdays are remote or in the community.

This schedule may be adjusted based on training, program, client, and community needs.

Salary

The starting wage is based on experience and company equity. Paid bi‑weekly. $21.03 - $31.56 per hour

Bilingual Language Differential Pay

Employees who successfully pass a language assessment are eligible to receive bilingual differential pay. This differential is provided as a fixed amount of $3,000 per calendar year, paid on a bi‑weekly basis across 26 pay periods, and prorated based on hire date and full‑time equivalency (FTE). Eligibility and payment are subject to the language requirements of the position and the employee’s demonstrated proficiency in the language.

Functions
  • Serve as payee for clients who require assistance to manage their finances.
  • Help clients in meeting basic needs for food, clothing, shelter, personal safety, and general health care, and assist them with applications for income, medical, housing, or other benefits which they may need and to which they are entitled.
  • Ensure clients' access to needed services and community resources by arranging for transportation.
  • Provide comprehensive psychosocial services designed to improve or maintain clients' abilities to function effectively.
  • Help clients evaluate strengths and symptoms, and facilitate clients in setting their own goals and plan for appropriate services.
  • Guide and instruct clients in daily living skills such as medication use, diet, exercise, personal hygiene, shopping, cooking, budgeting, housekeeping, use of transportation, and other community services.
  • Help clients develop social skills, interests, and leisure time activities, including opportunities for age‑appropriate activities.
  • Help clients find and make use of appropriate employment opportunities, vocational rehabilitation services, or supported work environments where appropriate.
  • Offer support, assistance, consultation, and education to families, friends, landlords, employers, community agencies, and others who come into contact with clients, to maximize benefits and minimize problems associated with the presence of these persons in the community.
  • Identify, work with, and make full utilization of potential natural support systems such as neighborhood networks, churches, and community organizations to encourage treatment program engagement.
  • Reach out to eligible clients, inform them of, and educate them about available services in the community.
