Licensed Counselor in Phoenix, AZ
We are seeking a Licensed Counselor to provide professional behavioral health services to decrease the impact of behavioral health disorders and other emotional disturbances. The primary responsibility is to provide assessment and treatment, which requires the application of specialized knowledge in behavioral health diagnosis and intervention. Work is conducted on an independent basis. Clinical supervision toward Board licensure will be provided.

- Determines client service needs by assessing individual client’s level of functioning, behavioral health symptoms and history
- Determine diagnosis and treatment needs
- Develop individualized client treatment plans and review frequently
- At each session, determines the degree of Danger to Self (DTS) behavior or intention of client
- At each session, determines the degree of Danger to Others (DTO) behavior or intention of client
- If needed, coordinates the petitioning of a client for a court-ordered evaluation
- After each session completes appropriate documentation
- Documentation must meet standards and be in compliance set by CPR’s Quality Management Department
- Coordinate and collaborate with all clinic support staff to assist clients with various needs
- Provide and follow-up on referrals when appropriate
- Completes discharge plan when clinically appropriate
- Other duties as assigned
- Current, unrestricted license issued by the Arizona Board of Behavioral Health Examiners: LMSW (Licensed Master Social Worker), LAC (Licensed Associate Counselor).
- Documented post-master’s experience for two (2) years, including experience within the past 24 months providing outpatient counseling services, or an equivalent of experience, skills, and education. Appropriate reference checks by letter or telephone as requested. Background check, as required by state.
$53,000.00 to $75,000.00 per year
