Position Summary

Unison Health is seeking a Case Manager to join our Home-Based Treatment (IHBT) team, providing services to youth and families with complex behavioral health needs. Small caseload – serve a limited number of youth and families at a time

Deliver intensive home-based case management services in accordance with state mandates and agency policies
Collaborate with clinical therapists to support individualized family service plans
Document all services provided and maintain accurate client records in the electronic health record system
Ensure compliance with confidentiality guidelines and proper handling of release of information

Education & Experience Requirements

Minimum of a bachelor’s degree in Social Work, Counseling, Psychology, or a related field
Must meet Ohio MHAS criteria for Qualified Mental Health Specialist (QMHS) if not licensed
Valid driver’s license and reliable transportation required
Must be insurable under the agency’s commercial liability policy
Experience working with children and families preferred
Must demonstrate knowledge in:
- Mental illness and treatment
- Individual and group counseling
- Behavioral health assessments and crisis intervention
