Clinical Community Liaison, Care Continuum

Overview

Find Your Passion and Purpose as a CARE Continuum Liaison, serving as a trusted guide for seriously ill and chronically ill patients and their loved ones.

As a CARE Continuum Liaison, you will work within the Care Continuum to support seriously ill and chronically ill patients and their families as they navigate complex healthcare decisions and transitions. In this role you will help individuals understand their care options, identify appropriate additional levels of care, and move through critical moments with compassion, clarity, and confidence. You will collaborate closely with clinical teams, community partners, and internal stakeholders to ensure patients receive the right support at the right time.

• Provide compassionate guidance to patients and families during healthcare decision‑making and care transitions.
• Assess patient needs and recommend appropriate levels of care (home health, hospice, palliative care, etc.).
• Collaborate with interdisciplinary teams to coordinate seamless transitions and continuity of care.
• Maintain accurate records of patient encounters, care plans, and service referrals.
• Advocate for patients and families within the healthcare system.

• Current licensed RN, APRN, MSW, or LPN/LVN.
• Minimum 5 years of direct clinical care experience in a community‑based setting.
• Experience in hospice care is required; home health experience is preferred.
• Knowledge of CBT, motivational interviewing, mindfulness, and solution‑focused interventions.
• Tech‑savvy: comfortable using multiple computer platforms and learning new systems.

Salary range: $85,000–$100,000 per year.

Field‑based position supporting the Chicago, IL market.

Hybrid role – work from home up to 3 days per week, with up to 2 days per week spent in the office or in the field supporting patients with transitions.
