Physical Therapist; PT - Home Health; Per Diem

@Home Therapy is seeking a Physical Therapist to provide in-home rehabilitation services to adult patients throughout Yonkers. This role focuses on helping individuals maintain mobility, manage ongoing physical limitations, and improve functional safety in their home environments.

This location offers enhanced earning potential and is well-suited for therapists looking to build a steady, community-based caseload.

• $500 bonus after 50 completed visits.
• Additional earning incentives available for this location.

• Evaluate balance, gait, strength, range of motion, and functional mobility in the home environment.
• Identify environmental and physical barriers that may affect safe mobility and independence.

• Develop and implement individualized treatment plans to improve or maintain mobility, strength, balance, and functional independence.
• Provide rehabilitation for patients recovering from illness, injury, surgery, or managing chronic conditions.

• Train patients in safe transfers, ambulation, stair negotiation (when applicable), and household mobility.
• Educate patients and caregivers on fall prevention, safe movement strategies, and the use of assistive devices.

• Monitor patient progress, reassess functional status, and modify treatment plans based on clinical findings.
• Collaborate with physicians and the interdisciplinary care team to support patient goals.

• Complete timely documentation using the agency's electronic medical record (EMR) system.
• Maintain compliance with clinical, regulatory, and agency documentation standards.

• Flexible Scheduling: Choose your availability and manage your caseload independently.
• Community-Based Practice Model: Provide care directly in patients’ homes across Yonkers and surrounding areas.
• Operational Support: Administrative coordination and scheduling assistance provided.
• Scalable Caseload: Opportunity to increase visit volume based on preference and availability.

• Active New York State Physical Therapist license.
• Experience with adult or geriatric populations preferred.
• Strong clinical reasoning and functional mobility expertise.
• Ability to work independently in a home care setting.
• Effective communication and patient engagement skills.
