People Care Health Services is seeking a Care Coordinator to manage client caseloads and ensure quality home care services across Southern Colorado. In this role, you will conduct assessments, coordinate care with caregivers and families, and support client needs effectively.
The ideal candidate will have experience in healthcare or caregiving, strong communication and organizational skills, and be comfortable working both independently and in a team environment. This position requires travel up to 50% of the time.
