Position: Remote Social Worker (LMSW) - Texas Child Health Access Through Telemedicine (TCHATT)

UTHealth Houston Department of Psychiatry & Behavioral Sciences – Texas Child Health Access Through Telemedicine (TCHATT) Social Worker (LMSW)

We are hiring a Social Worker (LMSW) to join our team as a care coordinator for TCHATT. The role manages patients’ transitions, coordinates care, and refers patients to agencies or health care professionals as needed.

The position also includes educating staff on case management, evaluating patient outcomes, maintaining documentation, and providing presentations to community providers on psychosocial factors that affect care.

Remote work is available, but employees may be required to attend trainings or meetings at the Greater Houston office.

Position Summary

Deliver professional social work services to assist patients and families with long‑term care and community resources while patients receive mental health services through the TCHATT program.

Position Key Accountabilities
  • Provide professional social work services to connect patients and families with long‑term care and community resources.
  • Plan treatment courses or refer cases to appropriate professionals, institutions, or agencies; manage patient transition through the TCHATT system and coordinate care.
  • Obtain patient data to support development of teaching endeavors, programs, and social services.
  • Assess patient and family needs; collaborate with physicians and the multidisciplinary team; develop patient goals, monitor progress, address barriers, evaluate outcomes, and conduct individual or group counseling.
  • Prepare detailed reports and videotape sessions for review with coworkers or faculty.
  • Solicit assistance from religious, social, and welfare agencies; interact with social work staff and psychiatric residents.
  • Educate physicians, nurses, and other providers on community resources and psychosocial impacts on care.
  • Apply crisis intervention, conflict resolution, and mediation to address patient/family concerns.
  • Respond to emergency situations.
  • Perform other duties as assigned.
Certification / Skills
  • Licensed Baccalaureate Social Worker (LBSW) by State of Texas is required.
  • Licensed Master Social Worker (LMSW) by State of Texas is preferred.
  • Bilingual is preferred.
Minimum Education

Bachelor’s degree in Social Work or a related field is required.

Minimum Experience
  • At least one year of social work experience is required; medical social work experience is preferred.
Physical Requirements

Exert up to 50 pounds occasionally, 20 pounds frequently, and 10 pounds constantly to move objects.

Security Sensitive

This position is security‑sensitive pursuant to Texas Education Code and relevant statutes. Employees may be subject to periodic background checks.
