Vaya Health LME/MCO is hiring a North Carolina Licensed Post Transition Coordinator to support individuals in maintaining their homes as part of the Transitions to Community Living program. This full-time position is remote from North Carolina.
The ideal candidate will manage post-transition planning and coordinate rehousing efforts for members at risk of losing their housing. A Master's degree in a Human Services field and valid North Carolina clinical licensure are required.
