The Ohio State University Wexner Medical Center Home Care seeks a licensed Physical Therapist to deliver home-based therapy under physician guidance. You will assess patient needs, develop plans, and monitor progress in patients' homes within the service area.
You will join an interdisciplinary team, engage with families, and coordinate with schedulers to ensure timely admissions. Ohio PT license and at least 1 year acute care experience are required or preferred.
