Overview

Behavioral Health Navigator III at Sentara Careplex in Hampton, VA. This is a full-time, permanent position (40 hours) with standard working hours from 8:00 am to 5:00 pm EST, Monday through Friday.

The Behavioral Health Navigator III serves as a senior member of the care coordination team, providing expert support for high-acuity behavioral health cases. This role includes workflow leadership, mentoring, and project participation to enhance service delivery across the care continuum.

Work shift:

• Education:
  
  Master’s Degree in Social Work, Counseling, Clinical Mental Health Counseling, Marriage and Family Therapy, Psychology, Addiction Studies, Nursing, or a closely related field OR Bachelor’s of Science in Nursing for RN candidates.
• Experience:
  
  Eight years of healthcare experience delivered in one of the following settings: acute care, long-term care, home health, hospice, skilled nursing facility, or community-based program services.

• Basic Life Support (BLS) – American Heart Association (AHA). RQI required within 90 days of hire.
