Join our team as an E-Referral Specialist and be the first point of contact in our referral process, ensuring seamless continuity of care for clients and referral sources.
Shift/Availability Requirements- Part‑time, 29 hours per week.
- Days/hours of availability: Tuesday & Wednesday 12:00‑5:00 p.m. EST, Thursday & Saturday 8:00‑6:00 a.m. EST.
- Work 2 holidays per year; additional holiday coverage may be required.
- Monitor multiple e‑referral systems to identify new referrals, manage alerts and notifications.
- Check email for requests to pull referrals and perform follow‑up communication.
- Scan documentation to determine patient eligibility for services (service area, orders, insurance, etc.).
- Track status of all referrals, follow up throughout the process, and report weekly.
- Communicate with branch, sales, and leadership via email or phone for status updates and referral decisions.
- Daily manage the ERST log with all referrals and ROC decisions.
- Maintain productivity levels as defined by departmental standards.
- Comply with all regulatory compliance areas, policies & procedures, and company best practices.
- High School Diploma or equivalent.
- Typing speed of at least 40 wpm.
- Basic math skills for data analysis and report generation.
- Proficiency with Microsoft Office (Word, Excel, Outlook, Teams).
- Logical thinking and decision‑making skills.
- Ability to follow strict timelines.
- Communication with all levels of management.
- Multi‑screen/systems proficiency with attention to detail.
- Time‑management skills in a fast‑paced environment.
- Fluent English reading, writing, and speaking.
- Reliable attendance.
- Travel requirement: 0%
- Experience with e‑Referral portals and EPIC systems.
- 1+ year customer service or healthcare‑related experience; preference for home‑health environment.
- Knowledge of home health service delivery issues.
- Working knowledge of Medicare enrollment and home‑health agency guidelines.
- Associate’s degree.
Home or hybrid employees must provide internet with a minimum download speed of 25 Mbps and upload speed of 10 Mbps. A dedicated, interruption‑free work space is required to protect HIPAA information. Occasional travel for training or meetings may be necessary.

Pay Range$40,000 – $52,300 per year (full‑time reference).
