Licensed Clinical Social Worker (LCSW)

Aviata Health Group is looking for a Licensed Clinical Social Worker (LCSW) to join our family. You will work with the geriatric population in a Skilled Nursing and/or Assisted Living setting, collaborating with nurse practitioners, psychiatric nurse practitioners, psychologists, and podiatrists.

• Provide psychotherapy and support to skilled nursing residents.
• Work within a balanced weekday schedule with no nights, weekends, or off‑hours required.
• Contribute to an integrated care team and consistent referral flow.
