Center Well Home Health is seeking a Speech Language Pathologist to deliver home-based therapy focused on rehabilitation and functional improvement. You will evaluate patients, develop care plans, and coordinate care with the clinical team to restore independence and enhance quality of life.
The role requires ASHA CCC or equivalent, at least six months of SLP experience, a current license and CPR certification, and a valid driver's license for travel to patients' homes.
