Join a traveling Physical Therapist team serving home health patients in Forrest City, AR, and provide evaluations, reassessments, and start‑of‑care visits that support patient recovery and chronic condition management. This role emphasizes thorough documentation, timely charting, and efficient delivery of high‑quality home‑based physical therapy services.
Minimum Requirements- Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
- Active Arkansas PT license required to start the assignment.
- Current CPR/BLS certification from AHA or ARC.
- Home health experience required.
- Experience with Homecare Homebase (HCHB) required.
- Ability to complete documentation within 24 hours is preferred.
- Efficient documentation practices and experience meeting productivity expectations in home health settings.
- Experience performing PT evaluations, reassessments, SOC visits, and treatment visits in patient homes.
- Shift hours: M‑F 8:00 AM - 4:30 PM.
- Weekly hours: flexible; open to 32‑40 hours per week.
- No on‑call or weekend requirement.
- Mileage reimbursement provided between visits, from first patient to last patient.
- Coverage area includes Phillips County and Helena/West Helena area. Candidates should be prepared to travel within the coverage area.
