Location: Vancouver, Clark County, Klickitat County, and Skamania County, Washington

Hybrid: Associates are required to be in‑office 1–2 days per week, with flexibility for remote work. Candidates must be within a reasonable commuting distance or qualified for a law‑required accommodation.

Role

Summary:

The Outreach Care Specialist Lead ensures that complex patient treatment plans are followed, proactively improves member health outcomes, and meets quality goals.

• Coordinate follow‑up care plans by scheduling appointments or enrolling members in programs.
• Assess member compliance with medical treatment plans via telephone or on‑site visits.
• Identify barriers to plan compliance and coordinate resolutions.
• Recognize opportunities that impact quality goals and recommend process improvements.
• Recommend treatment plan modifications and determine need for additional services in partnership with case management and providers.
• Coordinate identification of and referral to local, state, or federally funded programs.
• Coach members on ways to reduce health risks.
• Prepare reports documenting case and compliance updates.
• Establish and maintain relationships with agencies as contracts require.
• Serve as a team lead.
• Participate in cross‑functional teams on projects, initiatives, and process improvement activities.

• High‑school diploma or equivalent and 5 years of related experience, or an equivalent combination of education and experience.

• Experience in behavioral health or mental health.
• Certified nurse assistant or certified medical assistant.
• Bachelor’s degree in a related field.
• Bilingual candidates.

Salary Range: $30.00 – $45.00 per hour (URAC accredited areas).
