Center Well Home Health is seeking a Home Health Physical Therapist to help patients regain strength in their homes, reporting to the Clinical Manager. You will deliver patient-centered PT aimed at functional improvement and independence.
Responsibilities include planning and administering home-based therapy, assessing patients' progress, coordinating with PTAs and home health aides, documenting care, and communicating with physicians and families. 40 hours per week, competitive benefits.
