Sentara Health is seeking a Full Time Care Coordinator to serve Roanoke, VA and surrounding areas. The role requires travel for in-person assessments, with the option to work remotely on days without assessments, and full-time hours (40 hrs/wk) during day shifts.
Applicants must be located in Roanoke, VA and hold a Bachelor’s degree in Health & Human Services or a related field. Prior experience in social work or care coordination is required, with preferred background in /DD waiver services
