Supplemental Health Care is seeking a travel Home Health Physical Therapist for Fernley, Nevada. The position starts ASAP on a 13-week contract, offering 8-hour shifts (40 hours/week) with 5–6 patient visits daily. You will perform initial assessments, develop care plans, and provide skilled interventions in patients’ homes.
Requirements include an active NV PT license, current BLS, and 1+ year of home health experience. EMR experience with Homecare Homebase, Net Smart, or Epic is preferred.
