VNA Care is seeking a Home Health Social Worker (LICSW or LCSW) for the North Shore region. The MSW candidate will join an interdisciplinary team to complete psychosocial assessments in patients' homes and develop individualized care plans with community resource linkage.
Responsibilities include coordinating short- and long-term support with the patient’s primary care physician, offering short-term counseling when appropriate, and promoting home-based quality care.
